Reasonable and Customary Fees

This is the fee that a particular medical plan will approve for a specific test or service. Reasonable and customary fees are usually based on the average fee charged by a particular physician within a certain area. If the actual bill is more than the approved fee, the insured will need to pay the difference out-of-pocket.
